Sometimes pain occupies all inner space. It narrows attention, restricts movement, and makes the body listen only to a single signal.
But then music begins to play.
Breathing becomes steadier. Fingers find the rhythm. The person starts quietly humming along—and pain no longer seems like the only event happening inside.
Music does not always eliminate its physical cause. However, it can change the way the brain and body experience pain.
A new scientific review shows that not only the melody itself matters. Emotional response, the ability to choose music independently, and active participation—singing, moving, or playing an instrument—are also important.
Why does music help?
September 3, 2026 Drexel University presented a detailed analysis of a peer-reviewed study A scoping review of music-based pain treatment mechanism research. The review itself was published in the August issue of the journal PAIN Reports, and the research network Music4Pain reported on it on August 3, 2026.
The international team was led by Joke Bradt , a professor at Drexel University, a board-certified music therapist, and the director of the Music4Pain network. The project is funded by the National Center for Complementary and Integrative Health at the U.S. National Institutes of Health and studies the mechanisms of music's effects on pain.
The researchers considered not only whether music helps, but also tried to find out why it helps and under what conditions its effect can become stronger.
The team examined more than 600 scientific publications, but only 57 studies met the review criteria.
The analysis identified three particularly promising factors:
- positive emotions arising from listening;
- a sense of autonomy and control when a person chooses the music themselves;
- synchronization of movement with music—for example, tapping to the rhythm.
It is here that music ceases to be background and becomes action.
Pain is not born in a single point
Pain is not a simple signal that travels directly from a damaged part of the body to consciousness. Its intensity is influenced by attention, emotions, expectations, memory, anxiety, a sense of safety, and the ability to control what is happening.
Therefore, two people can experience similar physical sensations differently. Moreover, one person's perception of pain can change depending on their state and surroundings.
Music interacts with several levels of this experience at once. It can direct attention, evoke pleasant memories, reduce tension, and restore a sense of inner order.
But the review's authors suggest looking deeper: music may help not only because it distracts.
When a person chooses a composition themselves, they gain the ability to influence their own state. For someone whose life is limited by pain, even a small return of a sense of control can be significant.
Favorite music works differently
The researchers did not find a universal "melody for pain."
Music chosen by a doctor or experimenter may not evoke an emotional response. Even a piece commonly considered relaxing may bring peace to one person, but seem irritating to another or awaken unpleasant memories.
Therefore, not only tempo and a conventionally "correct" frequency matter. Personal meaning, pleasure, and how much a person likes the specific music are significant—researchers denote this factor as musical preference.
A favorite melody can return a familiar inner space. In it, the voices of loved ones sound, memories of home, a sense of freedom, experienced love, or a moment when the body was strong and light.
It reminds us: within us exists not only pain.
Why it is important not only to listen
One of the most interesting directions has been the study of active participation in music.
Joke Bradt notes that in clinical practice she uses singing, vocal improvisation, and playing instruments. According to her professional observations, active music-making can be especially valuable for people with chronic pain. However, scientific studies directly comparing different forms of such participation are still insufficient.
In one of the studies included in the review, joint singing produced a more pronounced result than passive listening. But this is a separate observation that does not yet allow asserting that singing is always more effective for every person.
During singing, many processes connect simultaneously:
- breathing acquires a certain rhythm;
- attention follows the melody and words;
- the voice is created by the movement of respiratory muscles and is felt by the body;
- a person independently controls the strength and duration of the sound;
- emotions get the opportunity to be expressed.
Singing is music that we not only hear. We create it with our breath and feel it from within.
Not a magic cure
The researchers emphasize important limitations of the available data. Especially few rigorous studies are devoted to people living with chronic pain for a long time. Many experiments were conducted with healthy volunteers or modeled short-term pain sensations in the laboratory.
Mechanisms identified in acute pain may operate differently in long-term conditions.
Moreover, the perception of music is influenced by genre, volume, familiarity with the piece, memories, cultural environment, and individual sensitivity. Pain itself is no less complex.
Therefore, music should not be seen as a substitute for diagnosis, drug therapy, or medical care. It can be used as an additional tool, especially under the guidance of a qualified music therapist, but its effect depends on the condition and needs of the individual.
The significance of the new review is not in promising healing with a single song. Music does not make pain disappear—it creates a space within a person where pain ceases to sound solo.
When a person again becomes the author
Perhaps one of the main features of music is not that it silences pain. It returns to a person the ability to hear the melody within themselves.
When we choose a melody, take a breath, and begin to sing or move, the body ceases to be only a place of suffering. It again becomes a space capable of creating sound, holding rhythm, and expressing life.
Music does not cancel what is happening, but it changes a person's relationship with it. The inner position gradually transforms:
"This is happening to me" turns into "I am doing this."
And sometimes relief begins right here—not when music simply sounds nearby, but when the person themselves enters its movement.
The body not only listens to music. It remembers that it too can sound.
